The Host Family Application form can be completed online. The Background Check is required for individuals age 18 or over in the home and can be completed by accessing the secure link below. Please phone your ETC representative for the password to complete the check, or call the ETC office at 1-877-222-9803 to complete the check for you.
Printable forms require Adobe Reader. If this application is not already installed on your computer, you may download it free of charge from Adobe.
